Membership Cancellation Form

    We're sorry to see you go!  To cancel your membership, please fill out the following form and hit the Submit button.  New York State law governs the following cancellation requirements.  All membership cancellations require 10 business days notice from the time of submittal to the time of cancellation.  Any payments that fall within those 10 business days will be charged and are not refundable.  Memberships may only be cancelled if  you have fulfilled your contracted number of payments.  Memberships that have not fulfilled the contracted number of payments can only be cancelled for one of the following reasons:  1.  Death of the member.  2. Member provides a physicians letter stating that the member cannot physically receive the club's services because of a significant physical disability for a period in excess of 3 months.  3. If the member moves their residence more that 25 miles from the health club and provides proof.  4. Cancels within 3 business days of the sign up date.
